Before participating with the Dobyns-Bennett Band, all students are required to submit certain forms related to contact and health information. Please print the forms below. Upon completion they may be dropped off in the mailbox in the lobby of the Instrumental Music Building.

2012-2013 Band Volunteer Survey

The Dobyns-Bennett Band needs parents’ support to continue its tradition of excellence. Last year the Dobyns-Bennett Band Boosters logged over 30,000 hours of volunteer time in support of our nationally recognized band. Please give us a little of your time this year. To volunteer, download the 2012-13 Volunteer Survey and consider volunteering your time to assist the band. Print and/or e-mail the completed survey to Shannon Taylor (address and e-mail is indicated on form). You won’t regret one minute of you time put into the support of the Dobyns-Bennett Band. No matter how small or large your commitment can be, please make that commitment.

Permission Slip

We have developed a Permission Slip for the convenience of our band parents. It includes the guidelines for obtaining approval for your child to not return along the entire group. You are not required to use this specific form, but must follow all the guidelines it contains.
